Electronic Signature Disclosure and Consent Agreement
CONSENT TO ELECTRONIC SIGNATURES AND DOCUMENTS
Federal law requires your consent to use electronic signatures and electronic versions of disclosures, contracts and other documents and records (“electronic documents”) which would otherwise only be valid if they were in writing. By completing and delivering this consent form to First State Financial (hereafter referred to as the bank) you acknowledge receipt of this consent form and consent to the use of electronic signatures and the use of any electronic documents used in Checking and Saving Statements involving you, the bank and other related parties. Any electronic documents delivered to you will be provided electronically online and an e-mailed notification will be sent to you at the e-mail address you are providing. If you wish to obtain a paper copy of any of the electronic documents you may telephone, fax or email at the address below with your request. Paper copies of current and future statements will be provided to you at no charge in the event you opt out of electronic statements.
You may withdraw your consent to use electronic signatures and electronic documents by contacting the bank via telephone, fax or through the e-mail address provided below. If you decide to withdraw your consent, the legal validity and enforceability of electronic documents and signatures used prior to the withdrawal will not be affected.
MINIMUM HARDWARE AND SOFTWARE REQUIREMENTS
The following hardware & software are required to access (open) and retain (save) the electronic documents:
Computer with an Internet connection.
Web browser that includes 128-bit encryption (e.g. Internet Explorer version 6.0 and above, Firefox version 2.0 and above, Chrome version 3.0 and above or Safari 3.0 and above) with cookies enabled.
Email: Access to a valid email account
Software/Electronic Document Formats: Adobe Acrobat Reader version 8.0 and above to open documents in PDF format.
Sufficient storage space to save past communications or an installed printer to print them.
We will notify you if there are any material changes to the hardware or software needed to receive electronic Communications from us. By giving your consent you are confirming that you have access to the necessary equipment and are able to receive, open, and print or download a copy of any Communications for your records. You may print or save a copy of these Communications for your records as they may not be accessible online at a later date.

CHANGES TO YOUR CONTACT INFORMATION
You should keep the bank informed of any change in your electronic or mailing address. You may contact the bank at the mailing address, fax or telephone number provided below regarding any such changes.
